Summary0001811: Remove xdebug.collect_includes setting and always include them
Description

This setting is on by default, and makes sure that included files are collected. Remove the setting and always include include filenames.

This is also a good opportunity to see if we can get away with not duplicating these file names and just add references to the zend_string that they are made of.
This seems possible by using zend_get_executed_filename_ex instead of zend_get_executed_filename.
